Birch Avenue Nursing Home provides 40 nursing home beds for people with complex dementia.

The Registered Provider and owner is South Yorkshire Housing Association, and we work in partnership with Sheffield Health and Social Care Foundation Trust.

The focus of the home is improvement and empowerment for the service user.


We are looking for a pleasant and friendly person to join the team, to work alongside and, at times, alone, with a good hold of communication and telephone skills to maintain a high degree of confidentiality, relating to our residents, in accordance with Trust policies and procedures.


Birch Avenue has embarked on an exciting journey and are seeking someone who enjoys working with older adults with dementia and would like to develop their skills in supporting people to lead meaningful lives and be a key member of the team to focus on positive well-being.


The applicant will, preferably, be a driver who is flexible and willing to work unsocial hours to ensure those under our care lead fulfilled lives which will also include delivery of personal care with others in our team.


The main duties of the role include:

  • Being primarily responsible for the arranging, coordinating, booking, and engaging our residents in meaningful activities both within the designated environment and externally.
  • Working closely with other staff to develop a tailored activity programme that will enrich the lives of our residents.
  • Arranging transport with trusted links to enable attendance at outside venue arranged activities.
  • Role model focused behaviour to support individualised care plans, supporting positive wellbeing, physical and psychologically.
  • To ensure appropriate risk assessments are carried out for all activities planned within health and Safety legislation, recording as appropriate.

A few examples of B3 Activity Coordinator duties expected are as follows:

  • Transporting residents to support social inclusion and meaningful activity.
  • Ensuring all appropriate risk assessments are completed relating to the activity, transport, service users and staff.
  • Ensuring that all equipment to be used is fit for purpose and all appropriate documentation is available.
  • To maintain privacy, dignity, and confidentiality of residents at all times.
  • To work as part of the team, providing practical support to residents, including personal cares. This will also include moving and handling, dressing, bathing, toileting and ensuring appropriate dietary intake is reached.
  • To report any changes or cause for concern relating to residents physical and mental health to the nurse in charge and management team to ensure appropriate care and treatment is sourced.
  • Documenting all actions taken on resident's records.
  • Liaise closely and appropriately with personnel and agencies internal and external.
  • Attend and participate in appraisals, supervisions, and team meetings.
  • To undertake appropriate training in respect of job requirements, personal and professional development.
