Job Summary
Mainly working with computer systems such as RIO and CM2000 software.

The successful applicant will manage the re-enablement care caseload, allocating work appropriately and efficiently, responding to changes in caseload delivery as required.

The care co-ordinator will be required to support patients with re-enablement care when the need arises, and will therefore need to be a car driver, with access to a car.

The Care Coordinator needs to possess excellent communication skills, and the role also involves line managing a team of staff with support from the Lead Care Coordinators, and the Clinical Lead.


This is a 30 hour post, the service runs from , you would be required to work a mix of Early and Late shifts, and alternate weekends.


Main Duties & Tasks


The Care co-ordinator will work as an active member of the Integrated Intermediate Care Hub and in partnership with therapy teams for Southern health Care Team for Southern Health to provide a high quality, efficient and effective service to patients.


The post holder will assume responsibility for the co-ordination and allocation of reenablementcare delivery, working within their scope of practice alongside the clinicaland administration team professionals.

They will work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

The Care co-ordinator will be proficient, knowledgeable and competent.

Educated to NVQ level 3/4, or equivalent, they will continue to develop in their role through the acquisition of further skills, knowledge and competences, to be determined by the needs of patients, the service and the team.


Qualifications
*NVQ level 3/4 or equivalent.

*Full Driving licence

£25,147.00-£27,596.00 per year
