The Great Yarmouth and Northern Villages PCH are looking for an enthusiastic Nursing Associate to join their busy team, which covers Great Yarmouth and the surrounding villages.

We welcome applications from experienced staff wishing for a change in direction and from new registrants who we offer a supportive environment to complete a robust Preceptorship programme.

You get to know your patients really well and give continuity of care because of caseload management.

Able to manage own time which gives you longer with the patients and their family

Main duties of the job

Community nursing offers a different experience to traditional ward based nursing; it does provide the opportunity to deliver a wide range of nursing interventions to people in their own home and within residential care. As a community nurse you will develop confidence and become expert in clinical skills including wound care and have the opportunity to deliver end of life care for people in their preferred place.

The community nursing service is commissioned to provide a 24/7 service. The core hours are between - hours, early and late shift patterns are also part of the working week. Applicants will be required to participate in working other shifts as part of the role including weekend and bank holiday working on a rotational basis.

Job responsibilities

Underthe supervision and working in partnership with a registered nurse provide anddeliver a high standard of nursing care contributing to the on-going assessment,provision and monitoring of care based on evidence and working in partnershipwith the multidisciplinary team.

Providecompassionate, safe and effective care and support to patients in a range ofcare settings.

Monitorthe condition and health needs of patients within their care on a continualbasis in partnership with patients, families and carers, ensuring appropriateescalation to a registered nurse when required.

Supportpatients to improve and maintain their mental, physical, behavioural health andwell-being.

Ensurepersonal and social needs are met and be able to care for a patients nursingneeds undertaking routine procedures including Venepuncture, ECG recording, Measuringand interpreting blood glucose levels, recording and interpreting vital signs,continence care, bladder and bowel care, pressure area care, wound care, nutrition,hydration, foot care, skin integrity.

Identifyand report changes in a patients condition to a registered nurse.

Prioritiseand manage own workload, recognising where elements of care can safely bedelegated to other colleagues, carers and family members.

Provideand receive sensitive information concerning a patients medical condition.

Workwith the registered nurse and engage with other agencies and community teams toensure a safe, individualised approach to the admission and discharge ofpatients.

Maintainaccurate, contemporaneous patient records in line with Trust policy and the NMCCode.

Provideeffective evidence based care for service users under the guidance of aregistered nurse.

Communication

Communicateeffectively with dignity and respect to all patients, relatives and othersgiving consideration to potential barriers to understanding.

Communicateeffectively with colleagues, providing clear verbal, digital or writteninformation and instructions when sharing information, delegating or handingover responsibility for care.

Reportaccidents, incidents, near misses or complaints following the Organisations policies/procedures.

Medicines Management andAdministration

Recognisethe effects of medicines, allergies, drug sensitivity, side effects,contraindications and adverse reactions.

Undertakesafe and effective administration and optimisation of medicines in accordancewith

Trustpolicy and the NMC standards of proficiency for nursing associates.

Administermedicines via the following routes, oral, topical, rectal, subcutaneous and intramuscularin line with NMC Standards and local policies.

Complywith the Organisations policy in relation to any safety critical medicines tobe administered

byNursing Associates.

Monitorthe condition and health needs of patients within their care followingadministration of

medication.Recognise and report any situations, behaviours or errors that could result inpoor care

outcomesrelated to medicines management.

Education & Development

Complywith mandatory competencies in line with the Organisations policy.

Toengage in a preceptorship period that includes achieving the Nursing AssociateMedicines Management Competency framework.

Maintainown continuing professional development in accordance with NMC continuing professionaldevelopment requirements and contribute to formulation of own objectives and personaldevelopment plan.

Support,supervise, teach (where appropriate) and act as a role model to trainee nursingassociates, student nurses, healthcare support workers and new staff members,promoting reflection and providing constructive feedback.

Contributeto the maintenance of the learning environment.

Adopt areflective approach to own practice with a view to continually improve.

Undertakefurther training as required, prior to carrying out any additional dutiesidentified as relevant to role, to comply with the Organisations policies andprocedures.
