The post holder will work as part of our Crisis team.As part of the role, you will work with individuals in crisis both I the community and within our Psychiatric Decision Unit, an alternative crisis assessment provision delivering crisis assessment and intervention.
The Psychiatric Decisions Unit is low stimulus, less clinical looking space designed to provide an environment and atmosphere that is conducive to enabling service users to engage with assessments, engage in therapeutic interventions and co-produce treatment plans.
This will enable the service user to both manage their immediate crisis and place them into a (usually community based) pathway, leading to more robust recovery.
The post holder with assist in the delivery of crisis assessment, risk assessment and planning and offer brief treatment interventions with the aim of reducing distress, reducing risk and improving mental wellbeing.
The post holder will have experience in mental health professional and will, act within their own professional boundaries will provide care for presenting service users from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
To work closely with health colleagues, non-mental health colleagues and a wide range of community services in order to facilitate a safe and effective plan of care.
To play an active role in safe discharge and ensuring all agreed post discharge plans are communicated clearly too all parties involved.
To support junior colleagues in delivering safe and effective person centres mental health care focussed on recovery.
To support fully the concept of collaborative and co-operative multidisciplinary/multiagency working in the team, supporting the process of partnership working across Rotherham Doncaster and South Humber NHS Foundation Trust with partner agencies.
Actively promote health education that may include providing alternative materials to support service users and carers in making decisions regarding their care and treatment.
To provide relevant support to carers and families in relation to their rights as carers and actively support their involvement in the care of the service user.
