    Job overview

    Come and join our well-established, proactive and supportive Physiotherapy Team in Glan Clwyd Hospital. The team works along the unscheduled care pathway working on increasing inpatient flow through the hospital and facilitating discharge. You will work along the entire pathway with a focus on admission avoidance, Frailty and the acute medical wards. You will have the opportunity to provide physiotherapy assessments and treatments of patients with a wide range of conditions including falls, frailty, neurology, and respiratory – including on call.

    We are looking for an enthusiastic, organised and caring individual with good communication and interpersonal skills. Effective and flexible team working is essential, as you will be working closely with a multi-disciplinary team to enhance patient centred care. We value the importance of continuing professional development, peer support and regular in-service training. You will be supported to assist and lead on service development by the Clinical Specialist and Physiotherapy management team.

    There will be opportunities to develop your leadership skills in both supervisory roles and through service development and potential for support in level studies.

    Main duties of the job

    To carry out Physiotherapy assessments and devise individualised therapeutic treatments plans. Work closely with the MDT to support complex discharge plans out of hospital.

    Within this position there is a good support, both clinically and managerially from a Team leader, Clinical Specialist and Advanced Practitioner.

    To ensure a high standard of care for Patients, linking in closely with families or carers, multi-agency teams and other disciplines involved.

    To communicate with patients, carers, agencies and other disciplines in an appropriate manner, often with sensitive information, diagnoses or outcomes.

    To supervise, teach and appraise peers, physiotherapists, physiotherapy assistants, technical instructors, students and other members of the multidisciplinary or multiagency teams.

    To train and supervise service users, families or carers and staff from other disciplines or agencies to carry out programmes of care devised by the therapist.

    To deputise and support the Clinical Team Leader as required in the daily management of the designated team.

    The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
