    Job summary

    Durham Coast PCN

    The Health & Wellbeing Coach will beresponsible for delivering group and one to one sessions on healthy eating,physical activity and health and well-being to meet with personalised careplans. The coach will play a critical role in engaging patients and use healthcoaching techniques to support them to take an active role in their health.

    You will work in a variety of venues in general practice and thecommunity. You will work collaboratively as part of our friendly personalisedcare team and also with our individual practices and with professional links. Teamwork, flexibility, enthusiasm, and the ability toprioritise tasks effectively are essential skills.

    We arecommitted to the ongoing development of our team, which includes mandatory anddevelopmental training for all our staff. Team members are encouraged tohighlight any training they feel would benefit them in the achievement of theirrole.

    Main duties of the job

    We are looking for an individual who will play a role in engagingpatients and use health coaching techniques to support them to take an activerole in their health and wellbeing. The Health and Wellbeing Coach isresponsible for delivering group and one to one sessions to improve patienthealth outcomes.

    The coach will play a critical role in engaging patients and encouragingthem on the benefits of making changes. This is a new role in our PCN and assuch the coach will be able to help shape and develop the role based on theirparticular passion such as, healthy eating, physical activity, improving mentalhealth etc, using health coaching techniques.

    The health and wellbeing coach will work closely with those of low tomedium complexity who will usually have 1 or more long term conditions.

    The post holder will have a key role in helping to raise the localpopulations awareness of the support groups and opportunities available to assistthem in achieving their health and wellbeing goals.

    As a whole, your role and skills will support and encourage theprevention of developing further illness, or the deterioration of existinglong-term conditions.

    Job responsibilities

    Identification of people withlong term conditions and low knowledge skills and confidence to manage theirhealth and wellbeing

    Providing support (clinical ornon-clinical) to a cohort of patients who will benefit from proactive healthmanagement and care including being the single point of contact for the personor carer to simplify access and coordination of services

    Playing a health and wellbeingcoaching role; teaching and supporting patients/carers to understand and managetheir own conditions and maintain an independent lifestyle through healthcoaching techniques to encourage patient activation

    Supporting the development ofpersonalised patient care plans, liaising with the practice team, patient/carerand the complex care team as appropriate.

    Proactively supportingpractice targets for number of patients who have seen a health coach

    Proactively outreaching topatients on a regular and agreed basis

    Where appropriate take andrecord patient observations such as BP/weight/BMI and when trained carry outphlebotomy to assist.

    Playing an active role in MDTmeetings if required (regular practice meetings to discuss high risk and / orcomplex patients) by gathering information and being prepared to update theteam on patient progress towards goals etc. (as per their care plan)

    Coordinating patient visits toprimary care amongst the primary care team, ensuring efficient and effectivevisits for the patient cohort

    Map and connect communityactivities/ resources at a locality level including supporting the developmentof a network of community health champions.

    Support the delivery ofcommunity based public health initiatives such as physical activity, healthyeating and social connectedness.

    Build and maintain stronglinks with the voluntary sector, supporting the voluntary and statutory sectorto network and improve partnership working

    Contribute to keeping thecommunity directory up to date for their allocated area

    Support delivery of systematicself-care support plans for those with COPD, Diabetes, Pre-Diabetes, CVD andmultiple long term conditions.

    Managing the effective use ofavailable resources and services in order to provide constant standards of carein terms of both quality and quantity

    Adopting a multi-disciplinaryand multi-agency approach to care, ensuring that all aspects of the patientsneeds are met

    Understand when it isappropriate or necessary to refer people to other health professionals/agencies

    Ensure the patient record isaccurately updated and maintained

    Manage waiting lists ifappropriate

    Understand the barriers forindividuals/groups in accessing support in the community, and use this insightin developing community-based support, working as part of the wider socialprescribing model

    Promote the service within thePrimary Care Network, both for users and clinicians, building positive workingrelationships

    Contribute to and work withothers to organise awareness raising events for services that help supportpeople to improve their health and wellbeing

    Communicate effectively withcolleagues, patients and carers so that information is shared in order to meetpatients needs

    The post holder will have akey role in helping to raise the local populations awareness of the support,groups and opportunities available to assist them in achieving their health andwellbeing goals.

    Qualifications

    Essential

  • Educated to A level/BTEC
  • Be enrolled in, undertaking or qualified from appropriate health coaching training, covering topics outlined in the NHS England and NHS Improvement Implementation and Quality Summary Guide. The training to be delivered by a training organisation listed by the Personalised Care Institute
  • Evidence of continuing professional development
  • Desirable

  • Health/Social Care Degree
  • Experience

  • Adhere to a code of ethics and conduct in line with the NHS England and NHS Improvement Health coaching Implementation and Quality Summary Guide
  • Have formal, individual and group coaching supervision which must come from a suitably qualified or experienced individual.
  • Working closely in partnership with the Social Prescribing Link Worker(s) or social prescribing service provider to identify and work alongside people who may need additional support, but are not yet ready to benefit fully from social prescribing
  • Experience working in health and wellbeing, public or voluntary sector or leisure and fitness environment
  • Experience in a customer facing role
  • Experience delivering lifestyle change / brief interventions
  • Experience working in a health trainer / advisor role
  • Excellent people skills, interacting with all types of people and able to change communication style to fit the person
  • Ability to work independently and as part of a team
  • Comfortable working with people from public, private and voluntary organisations
  • Highly organised, managing time, resources and competing priorities in a structured and efficient way
  • Highly motivated and willing to go out of your way to help clients
  • Creative problem solver and willing to search for hard to find information
  • Good IT and administrative skills
  • Good understanding of health issues and the wider determinants of health and wellbeing
