Are you passionate about improving outcomes for children and young people? Do you have experience of working with children who have complex lives, across a range of organisations?


We are looking for an exceptional individual, with a successful background in working with and managing services for children who have highly complex health, education and social care needs.

You will work at the centre of a multi-agency approach to identifying needs and unblocking issues to ensure children access the right care and intervention at the right time and in the right place.

At the centre of a system-wide virtual children and family's team, you will work closely with a wide range of colleagues from across health, social care, education and the voluntary and community sector.


As part of the leadership teams in both Hampshire County Council and the ICB, you will provide leadership across the commissioning, management and monitoring of individual packages of care for children with high support and complex health needs and/or disabilities.


As a strong team player, you will be able to work collaboratively with key partners from health, education and children's social care, balanced with the ability to work autonomously when commissioning, co-ordinating and supporting the case management of individuals with highly complex needs.

This will be achieved through excellent working relationships with health, social care and education partners to ensure that the systems work for everyone, whilst remaining focussed on the child at the centre.
