    Description

    The post holder is a member of the Neuro Rehab team/ Early supported discharge Stroke service and will be responsible for the assessments, planning, implementation, and evaluation of individualised physiotherapy therapy intervention to stroke and neurological patients living in the Bedfordshire community.

    This will include range of community settings including patients home, care homes, rehab ward settings. This will include lone working within the community with access to additional therapist support when required.

    To undertake comprehensive assessments, develops and implements specialised physiotherapy treatments/programmes for patients with stroke or a neurological diagnosis.


    Works as part of multidisciplinary team including Occupational therapists, Speech & Language therapists, Psychologists and Assistant Practitioners in providing rehabilitation to Stroke and Neurological patients within the community.

    To undertake comprehensive assessments, develops and implements specialised physiotherapy treatments/programmes for patients with stroke or a neurological diagnosis.

    To use range of facts or situations as part of the assessment and provide appropriate physiotherapy from range of treatment options.

    To select and deliver evidence-based specialist physiotherapy treatments, agree goals with patients and monitor outcomes.

    To involve patients and carers in the planning of rehabilitation programmes and encourage self-management where appropriate.


    Provide adaptive equipment, rehabilitation programmes, and make referrals on to other professionals and agencies as appropriate to meet the identified needs.

    To plan and undertake the discharge of patients from Neuro rehab service when agreed goals have been reached or treatment is no longer appropriate (e.g. the patient has reached a plateau or there is a change in their medical circumstances).

    To be confident in their own diagnosis and be able to communicate that effectively to others (including medical professionals and other Allied Health Professionals).


    To share information in a timely manner with more senior therapists if there are concerns about a patients safety, condition or progress.


    To work a variety of shift patterns which may include weekends and bank holidays on a rota basis in the future.

    There may be occasions which are unpredictable, when the post holder may have to work late to support patients risk.


    Using verbal, non-verbal and written communication tools, ensure that patients and carers have an understanding of their condition and the role and aims of physiotherapy in relation to this.

    To communicate effectively with all members of the Bedfordshire Neuro Therapy Service and other providers to ensure optimum patient centred care.

    Patients seen will range from the vulnerable (e.g. elderly, mental health concerns or those with poor memory) to those who are extremely well informed about their condition.


    Responsible for equipment used in course of treatment/ handles patient valuables responsible for physiotherapy equipment used by patients & others orders supplies e.g.

    walking aids

    Responsible for day-to-day supervision and development of unregistered staff within Neuro and ESD service

    To oversee patient intervention and ensure that they deliver high quality patient care.

    Provide support and education to new staff and students.

    To participate in delivery of the team development plan and also contribute towards policy development/proposal.

    To continue to develop specialist Physiotherapy skills in a specific clinical field.


    To exhibit and maintain a high personal standard of professional expertise, attitude and behaviour at all times in accordance with Chartered Society of Physiotherapy, HCPC and local Trust, and service policies.


    Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.


    Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

    The post holder must adhere to the Trust risk assessment and risk management processes

    The post holder must adhere to infection control policies and procedures

    Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

    BSc,MSc or Diploma entry level qualification into the Physiotherapy profession.
    Member of the Chartered Society of Physiotherapy
    Member of Association of Chartered Physiotherapists Interested in Neurology

    A broad range of undergraduate placements completed including stroke and/or neurology
    Community experience - as undergraduate placement or in other services

    Clinical experience including neurology and/or stroke in different settings
    Detailed understanding of a wide range of medical conditions.

    Effective communication skills, written, verbal, report writing skills
