    An exciting opportunity has arisen to be part of the Shared Lives Team, as a Shared Lives Co-ordinator located in Derby city,
    The Shared Lives Service offers people the opportunity who need care and support to live a fulfilling life either through day support or living with a family.

    We are looking for someone who could make a difference to join our Team.

    As a highly motivated individual, you will be advising Shared Lives carers about delivering person-centred care and support to our customers either within the carer's home or within the community. An understanding of working to 'outcomes' would be an advantage. This will be in line with the requirements of Care Quality Commission (CQC).

    You will need to be an excellent communicator, have good organisational skills with the ability to support and advise carers to manage risks and promote successful outcomes for the customers within the Scheme. An understanding of Adult Safeguarding procedures and the ability to recognise and implement these in your role, is desirable – training will be provided.

    Part of the role will involve visiting Shared Lives Carers in their homes and obtaining feedback from customers using the Service.

    Experience of working with a wide range of customer groups would be an advantage.
