Norfolk County Council invites you to be part of our dynamic HomeFirst Hub team.

We're seeking a Senior Business Officer to play a vital role in ensuring safe discharges, reablement opportunities, and ongoing care planning for individuals in crisis and leaving hospital.

The HomeFirst Hub operates as an integrated team, collaborating closely with Norfolk Community Health & Care (NCH&C), NHS Acute colleagues, the Voluntary sector, and other Norfolk County Council colleagues.

Helping to support individuals on the Discharge to Assess pathway back home and assist those facing complex admission avoidance situations within the Network of Escalation Avoidance Team (NEAT).

This is a hybrid working post, with a blend of remote and office based working, currently from the Broadland Business park and Norwich Community hospital.

The team is operational 7 days a week:
Mon - Fri 08:00 - 18:00, Sat 09:00 - 17:00, Sun 10:00 - 16:00.
