Description

We are seeking a highly motivated, enthusiastic individual to join the West Surrey and North East Hampshire Community Team for Adults with a Learning Disability.

This is a rare and exciting opportunity for a committed individual to join a busy NHS community team.


This role offers the opportunity to participate in the delivery of health interventions under the supervision of qualified clinicians, to meet peoples health needs.

The successful applicant will work across a range of specialties under the guidance of nurses, speech and language therapists, occupational therapists, dietitian and physiotherapists.


This will involve practical work with people such as implementing skill teaching programmes to promote good health, completing observations and reporting back and creating resources that help people to understand their health needs.


As part of the role involves supporting physiotherapy and may involve water based therapy (aquatic therapy) There will be an expectation that you will undertake life saver training.

We also provide social care services for people with a learning disability in Croydon and ASD and ADHD assessment services in Hampshire.

We work closely with other NHS and voluntary sector organisations who provide services and support people who use services and carers.

Key Responsibilities

Clinical

  • To actively work with people in implementing the care plan devised by a team professional.
  • To act as an advocate and ensure that there is proper regard for peoples' dignity, choice, selfesteem, racial, cultural, sexual and religious needs and preferences.
  • To participate in assessments, planning, implementation and evaluation of interventions as part of a holistic approach working in a variety of
