    Job summary

    My job makes better lives by providing timely interventions that contribute to the care of young people with mental health problems and their families. I will put the experience of the CYP using the service at the centre of my decision making whilst building a supportive and responsive relationships with colleagues.

    The role of Mental Health Practitioner is to work in partnership with children and young people in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the wellbeing of children and young people and their families/carers. The post holder will assess, plan and implement care, and provide specialist advice and interventions. They will provide clinical supervision to staff and students and act as a mentor for students.

    You will have demonstrable expertise working with young people under the age of 18 years.

    You will be a Registered professional with current registration, appropriate to the job role: (Social work, Teachers, Occupational Therapists, Speech and Language Therapist and Creative Therapists)

    Main duties of the job

    The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of children and young people by undertaking the following duties:

  • To undertake assessments of individuals and their families in the designated care group with complex health presentations, including those children and young people presenting with higher levels of risk. To develop care plans alongside the child or young person.
  • To offer specialist assessment and advice where required.
  • To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the child, young person and carers. To regularly review risk factors and make changes to the management of them as necessary.
  • To contribute to the maintenance and development of the Trust Nursing and Mental Health Strategy.
  • To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.
  • To provide supervision of junior staff and trainees where appropriate.
  • To act within the professional body's code of conduct at all times.

    Job responsibilities

  • Carry own caseload and undertake holistic assessments including risk of the child/adolescent /family needs, either individually or in identified complex cases jointly with another member of the CYPS Community Team.
  • Provide short term treatment to CYP individually or Group based.
  • To liaise with Social Care, Education and other appropriate agencies/professionals /carers to ensure best outcomes for CYP.
  • Act as a lead professional for CYP following initial assessment. Developing appropriate plan of care.
  • Offer support for Students on placement.

  • Degree or diploma level qualification in relevant profession
  • Registered professional with current registration, appropriate to the job role
  • Evidence of continuous professional development
  • Experience

    Essential

  • 2-year post qualification experience of working in the community with young people with mental health difficulties and their families
  • Experience and ability to undertake Mental Health Assessments, Risk Assessments and use outcome measures to evidence good practice
