About the Role:


There is an exciting opportunity for a digital & transformation lead to join us at Hillton Primary Care Network, on a part time basis for 17.5 hours per week, to lead us as we transform the way that we support our patients, the way our staff are able to work and the sustainability of our general practice services, through the adoption or optimisation of new technology and other initiatives.


The responsibilities of the post-holder will include:

  • Overseeing the adoption and/or optimisation of new technology to enhance patient access and experience, and to increase PCN productivity
  • Building relationships and facilitating collaboration between practices and the wider system, to support the delivery of care to patients (including shared appointments between practices to aid delivery of enhanced access)
  • Leading an improvement approach to change, including building capability for quality improvement within the PCN, and systemwide approaches to problem solving;
  • Reviewing and improving the PCN's digital maturity
  • Using data, and improving data quality, to:
  • understand demand, capacity and activity, and to drive improvements in:
- patient experience of access
- operational efficiency including better matching capacity to need
- staff experience at work
- support population health management
- support understanding of the type and intensity of support and training needs of the PCN, and coordinate this support, including through OD programmes
- facilitate clinically-led innovation and the effective adoption of improvement initiatives, including integrated working at neighbourhood and/or place level to improve access to services for patients.

  • Educated to master's degree level, or equivalent professional experience
  • Experience in digital/IM&T project management delivery through the entire project lifecycle from initiation to delivery
  • Good understanding of the digital agenda within the NHS and social care/care sector
  • Good understanding of current healthcare policy, relating to primary care and integrated care
  • Ability to work on own initiative and organise workload, allocating work as necessary
  • Used to working in a busy environment
  • Adaptability, flexibility and ability to cope with uncertainty and change
  • Ability to deliver a vision and a willingness and drive to adapt to NHS targets and business planning, whilst keeping abreast of legislative and contractual changes
  • Selfmotivation and forwardthinking approach
  • Excellent organisational, management and planning skills, IT skills and communication skills
  • Ability to implement and manage change effectively.
