Job summary
An exciting opportunity has arisen for a PCN Admin Care Coordinator to join our ever-expanding team within Tadcaster and Selby Rural PCN working alongside General Practice teams.
Tadcaster and Selby Rural (TaSR) PCN are a well established 3 Practice PCN with approximately 32,000 patients, covering the large geographical area surrounding Tadcaster, Sherburn-in-Elmet and South Milford.
The right candidate should be caring, dedicated and compassionate. They will also need an open and flexible attitude towards working with a variety of different patient needs. Good written and verbal communication skills as well as strong time and case management abilities are also needed.
The Admin Care Coordinator will play an important part in supporting the running of the PCN through various ongoing projects and workstreams alongside the PCN Manager and Digital Transformation Lead. Key areas of focus will be
Main duties of the job
The PCN has a sizeable cohort of patients who require additional special attention and support in the booking and attendance of their annual reviews (LD and SMI). Part of this role is to actively encourage and engage the patients with the appointment booking, reminders and follow up calls. Patients are recalled within their birth month and the work follows a process of what each patient requires.
The PCN also wishes to support the attendance of patients to NHS Health Check appointments as part of individual Practice targets. The role supports the identification of patients through clinical reporting. The role works closely with the Clinical Care Coordinator.
The role may also provide additional capacity to support the booking of the Covid vaccination program as well as any other national vaccine programmes.
The role will be responsible for the triaging element of the social prescribing service, directing patients to the most appropriate service whether that be in house or externally.
The Admin Care Coordinator will work closely with other PCN staff, clinicians and AHP's to support the administration of the local care homes as well as any other projects at the request of the PCN Management team.

South Milford Surgery & Bramham Medical Centre are part of the same partnership but the surgeries have separate patient lists and are members of different primary care networks.
