About the role

If you are passionate about unlocking the potential of young people and supporting them to be the best that they can be, we are looking for you_
At Swan we have a dedicated Care and Specialist Accommodation department which is made up of three key areas. These are Supported Housing, Home Care Services and Key Worker Living. We offer a variety of homes, schemes, services and accommodations to suit our customer's needs.

Within Home Care Services we are proud to offer Extra Care Schemes, Rapid Response Home Care Services and Care in the Community.

We have a team dedicated to providing continuous improvement and ensuring quality for both our customers and staff and we are proud to hold both outstanding and good rating with the Care Quality Commission.


Our Foyers provide a safe, caring place for young adults aged 18-25 years of age who need supported housing to live, learn and make the transition to independent adulthood.

We work with our young adults and the Foyer Federation to deliver a service that focuses on success, the young person's strengths, dreams and aspirations.

A team of coaches support our young people, teaching them life skills that will develop their independence and prepare them for moving on, including gaining education, training and employment

In this role you will play a part in enabling them to move on into successful and independent living. It will be your role to encourage them to lead flourishing lives.

Within this role you will be supporting and coaching our young people to:

  • Unlock their potential and encourage them to fulfil their dreams
  • Provide support to enable them to gain life skills to live independently
  • During their 2 year stay, being an advocate for them to own their own journey
  • Advise and support with education and employment
  • Roll out exciting weekly activities and events
  • Set up and maintain utilities
  • Complete budget plans
For the above post it is
essential that you will:

  • Have a minimum of 1 years' experience in a support worker role, working with Young People, or similar
  • Be an excellent communicator
  • Be passionate about working with young people
  • Be inspirational, creative & positive
  • Be proactive and manage your time productively
  • Be IT literate
  • Understand the importance of professional boundaries
  • Be able to complete case load paperwork within set timescales


The working pattern for this role is based on a four-week set rolling rota, consisting of early shifts 7.15 am pm, and late shifts 2.00 pm pm.

These shifts will include working every other weekend.
