    An opportunity has arisen for a Band 6 Care Coordinator working within the Early Intervention in Psychosis Team based at Parklands House in Basingstoke.

    The post holder will be from a mental health nursing background.

    You will need to be able to work flexibly and to demonstrate knowledge of the CPA process, risk assessment, and the recovery model.

    This post involves weekend and bank holiday working on a rota basis as per service user need.

    To provide specialist evidence based clinical interventions to young people and their families in conjunction with early intervention core team specialists and Support Time and Recovery workers and community partners.

    The transformation will happen in staggered stages Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024.

    The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

    Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

    Mental Health qualification as an Occupational Therapist, nurse, social worker

    Thorn/ BFT psychosocial interventions or equivalent (or willingness to undertake appropriate training)
    *Mentorship/ willing to complete training

    Experience supporting clients through crisis points in mental health
    *Relevant experience as a community care coordinator
    *Experience of working with young people with psychosis
    *Excellent verbal and written communication skills, including report writing and presentations
    *To have CBT certificate Training (or willingness to train)
