Dementia Care Assistant - London, United Kingdom - Homerton Healthcare NHS Foundation Trust
Description
We will be recruiting Dementia Care Support Workers to work within the the Elderly Care Units.
You will work closely with Registered Nurses, Dementia care lead and the multi-disciplinary team, delivering high quality care to dementia patients.
Due to government guidelines the Trust is unable to provide sponsorship for this role
Be able to establish useful and helpful relationships with patients who are confused, promoting their health and enabling them to function at their optimum level.
Respond positively to the patient's needs/care by offering support and reassurance.
Use appropriate, tactful and sensitive communication skills with patients and their relatives/care
Provide and receive routine information verbally and in writing to and from all members of the multidisciplinary team.
Continually assess care which is delivered to ensure that it helps to maintain independence.
Be aware of any risks to the patient in their environment and report immediately to the nurse in charge if any risks are
Observe patient's functioning to assess progress and stability in their mental health and report any changes to the nurse in charge.
Support patients with the therapeutic activities set up on the ward.
Carry out non-clinical activities and therapies as part of individual patient care
Plan and prioritise care with the nurse in charge
Take appropriate action when risks identified and complete unusual occurrence documentation
