    West Leeds Primary Care Network Job Summary

    To boost capacity within our specialised frailty and Home Visiting team, we need an experienced nurse or advanced nurse practitioner with a good breadth of general healthcare experience who can undertake acute home visiting across our network area. There will be a range of conditions to assess potentially but the most common reasons for daily acute visits are chest infections / respiratory issues, urinary tract infections and wound care issues. You will be part of a very experienced team and there will be plenty of debrief opportunities with other clinicians. You will also be assigned a clinical supervisor to help with your continued professional development.

    The successful recruit will be expected to work across all practices in the PCN, which means they will be working in the community across Pudsey, Bramley, Farsley, Stanningley and Calverley.

    Main duties of the Job

    One of the main purposes of this role is to alleviate pressure within our six member practices; we aim to undertake as much of the daily acute domiciliary work as possible across the network area so that the general practice clinical staff are freed up to do other things.

    Our acute home visiting service is multi skilled, comprising Advanced Clinical Practitioners, Physician Associates and a paramedic; these staff members also work in areas other than acute so they have a mixed remit. We need an additional member of staff to bolster overall capacity for this service; an experienced nurse will bring certain skills to the team that we don't currently have. We can be flexible with the hours – either part time or full time can be considered.

    Your home visits will be a mixture of acute undiagnosed issues, acute exacerbation of chronic condition/s and on occasion management of chronic concerns.

    You will visit the patient at home, take vital observations, take a history of complaint and examine if possible. You will not be expected to come up with a diagnosis although it would be helpful if you are able to. You will be expected to debrief the on-call GP following every patient to ensure best practice and the correct treatment plan is in place. Furthermore, daily debriefing will aid your professional development and build up your skill set.

    You may contact the patient via telephone and undertake the consultation that way where you deem a home visit unnecessary. You may also decide to refer on to a more suitable alternative clinician but you will be expected to make that clinical decision, document it well and explain your reasoning.

    In addition to the case by case GP debriefs you will also have the opportunity to discuss cases with your direct team mates as well as the wider PCN team. This in house MDT working provides true holistic and joined up care for our patient population. Some of your patients may require some complex care, particularly those with chronic conditions. You will be well supported with these cases, in particular by the very experienced frailty team

    You will attend weekly MDT meetings with a consultant geriatrician hosted by our frailty team to discuss the most complex patients who have regular home visits.

    You will visit acutely unwell patients in their homes, assessing their needs and treating them accordingly
    • You will work as part of a multi-disciplinary team in a patient facing role to assess and work with patients using expert knowledge
    • You will work with the wider PCN teams as appropriate where an onward referral is in the patient's best interest
    • You will maintain accurate clinical records of all patient consultations and related work.
