Practice Nurse with Special Interest in Diabetesrequired for 32 hoursa week for a large Practice in Kidderminster with 15400 patients.
Main duties of the job
We are looking for an enthusiastic individual tojoin our practice nursing team. You will be responsible for the routine,evidence-based care of patients with Diabetes, including annual reviewsincorporating local prescribing guidelines as well as following up patientswith more complex diabetic needs including insulin transfers.

Role of the department: Assist GPs in the care of practice patients toinclude treatment, preventative care, screening & patient education.
Main Duties:
Clinical
Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with chronic diseasesor complex needs with the support of ourGPs.
Clinically examine and assess patient needs from aphysiological and psychological perspective and plan clinical care accordingly.
Proactively identify, diagnose and manage treatmentplans for patients at risk of developing a long-term condition as appropriate.
Diagnose and manage both acute and chronicconditions, integrating both pharmacological and non-pharmacological basedtreatment methods into a management plan.
Review medication for therapeutic effectiveness,appropriate to patient needs and in accordance with evidence-based practice andnational, local & practice-based protocols.
Work with patients to support compliance with andadherence to prescribed treatments. Provide information and advice onprescribed and over-the-counter medication regimens, side-effects andinteractions.
Support patients to adopt health promotionstrategies that promote healthy lifestyles and apply principles of self-care.
Carry out those activities involved when assessingpatients including but not limited to diagnostic tests such as blood tests,peak flow readings, spirometry and ECGs.
Recognise the significance of the observations madeand use these to develop a plan of care, with the support of the Clinical NurseManager.
Treatment of wounds, dressing and management of legulcers, removing sutures, routine injections, childhood immunisations & earsyringing where required.
Advising and giving immunisations and vaccinationsas appropriate for patients travelling abroad.
Taking and despatching laboratory specimensincluding blood, urine, swabs and cervical smears.
Recognise and work within own competence andprofessional codes of conduct as regulated by the NMC.
Administrative
Prioritise, organise and manage workload in amanner that promotes quality.
Produce accurate, contemporaneous and completerecords of all patient contacts, consistent with legislation and practiceprocedures.
Support and participate in shared learning acrossthe practice.
Liaison with and referral to other agencies.
Assist the management and admin teams in assessingand implementing new enhanced services and QOF requirements.
