We are looking for Navigational Support Workers with excellent people skills to assist with client discharge coordination & navigational support with social care services.

Responsibilities include liaising with community organisations, government agencies, families and carers, community services, housing departments, third sector organisations, and other relevant parties to ensure smooth transition to the community from hospital.


This requires you to work in a targeted way to achieve the best pathway out of hospital, maximising independence and choice for the individual and working in a solution focussed way to ensure that once ready, there are no barriers in place that would affect discharge.


This is a unique role that offers exposure to skills development across services, business functions and discharge coordination through means of providing adequate social care support.


  • The post holder will work with individuals through their attendance and participation at weekly Bed Management and Ward rounds meetings. This will provide them with a complete picture of the acute care pathway and barriers to patient flow.
  • The post holder must be adept at prioritising the activities that will most rapidly lead to effective discharge and have strong legal literacy to ensure that there are no legal barriers to discharge. This includes expert knowledge of housing legislation, No Recourse to Public Funds, Mental Health Act (s117) and the Mental Capacity Act.
  • The post holder will sit organisationally within the Inpatient Services and be present on the inpatient wards within a defined geographical area working with an identified caseload to enable effective discharge planning.
Attend ad hoc meetings with MDT/ Ward Managers/ Unit Coordinator/ Senior Nurse Manager where appropriate.

  • Resolve arising barriers to discharge i.e. keys, other agency support to discharge.
  • Ensure early liaison with the Recovery teams, CATT and the local authority to allow for adequate postdischarge support.
  • To maintain accurate, clear and concise clinical records and provide verbal and written reports to the clinical team and other relevant parties, adhering to local procedures and professional standards and codes of ethics as related to documentation.
To complete supervisee's PDP and inform line management of achievements, training and development needs identified
