Do you wish to be a part of a team striving to make a difference in our community? Do you want to make a difference in patient care?We are looking for a Band 5 Community Nurse to work within our Wisbech Team.
In this role, you would be working at part of the Neighbourhood team which includes District/Community Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurses and Team coordinators.
Linking closely with colleagues including general practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines.
You will need to hold a full UK driving licence* and have use of a vehicle. Have experience of working in the CommunityAre educated to RGN level 2
Are able to demonstrate a commitment to high quality care and service provision
Have the ability to assess and formulate care plans
Are able to teach and support student Nurses and Health Care Assistants
Keeping the patient at the centre of care, ensuring patient and carer participation in decision making
Providing a high standard of care
Providing a high standard of nursing intervention within a patient's own home, with access to Specialist and Advanced Practitioners when required

Appreciates the significance of safeguarding and interprets this accurately for all children and young people/ vulnerable adults whatever their life circumstances.
Responsible for maintaining registration with NMC and/or other professional bodies
Travel around work base locality
Manoeuvre patients using safe techniques in accordance with care plan.
