    Job summary

    This post is open to Registered Nurses (all branches), Midwives, and AHPs.

    The Lead Professional will be theclinical expert and professional resource for the management of children andyoung people with complex presentations related to: mental health, trauma,violence, or exploitation. They will manage the in-hospital youth workservice and lead on the development of a youth worker programme to support engagement with youngpeople aged 11-25, who present at the Emergency Department and/or are admitted to hospital following aviolent incident.

    Main duties of the job

    The lead professional will be at the forefront of this new service development; utilising project management and leadership skills, they will plan, monitor, and deliver on key performance indicators that can demonstrate the service's effectivness as well as demonstarte the ability to improve patient care and experience.

    Job responsibilities

    Clinical

    Develop a high level of clinical decision making,monitor and improve standards of care, working within their advanced scope ofprofessional practice.

    Contribute towards a multi-professional approachin the management of patients accessing the in-hospital youth work service.

    Support patients and their families and ensurethat they receive the required information to enable them to self-manage, lead,or participate in their care delivery.

    To support, assist and encourage themulti-disciplinary team in resolving patient problems, by the provision of newand innovative models of case management.

    Support the delivery of adjusted patientpathways for patients accessing secondary care.

    Provideadvice and support on the mental health act to ward staff and support thereceipt and scrutiny of Mental Health Act Paperwork.

    Leadership

    Bean innovative, enthusiastic role model providing leadership, guidance, andadvice to staff on operational and professional issues promoting an open andhonest and transparent culture.

    Provideprofessional visible leadership and a supportive approach that contributes tocreating a climate for achieving excellence in clinical practice, skills andcompetencies using the values of the Trust.

    Inpartnership with the Head of Complex Needs and mental health champion CYP, leadthe implementation of service improvements for young people with Complex presentationsand the evaluation/monitoring of care through audit, complaints monitoring, andincident monitoring and patient/carer feedback.

    Identify, develop, and sustain mechanisms tosupport patient involvement and feedback.

    Be a resource for health care professionals bybeing visible in the clinical area, available, and accessible for support andadvice in relation to the management of patients in the speciality.

    Provide leadership to the team and working withthe head of complex needs and mental health champion CYP, develop new roles /ways of working (High Impact Changes) linked to directorate business plans.

    Leadon consultations and develop strategies for delivering effective care within achanging environment that improves the quality of care and health outcomes.

    Management / Operational

    Employ effective decision-making skills toaddress complex issues and use effective change management skills to implementthese.

    Takethe lead in investigation of incidents / complaints when required, includingdetailed reports / letters to complainants.

    Work in partnership with the Head of ComplexNeeds and mental health champion CYP to manage the team towards achievingdirectorate, and Trust objectives.

    Identify and develop proposals and businesscases as required for future service innovation.

    Manageand develop and influence the development of practice / services in clinicalareas through the business planning process / service development and inconjunction with Matrons, Managers and Lead Clinicians.

    Delegate work to match capabilities and workloadof team members.

    Manage own and other resources effectively tobudget ensuring quality service delivery within budgetary constraints seekingto reduce budgetary costs by ongoing review.

    Monitor sickness, absence, and competence issuesin line with Trust Policy.

    Manage recruitment and selection of staff inline with Trust Policy.

    Advocate

    Ensure that high standards of clinical care aregiven and maintained and act when standards are not being met.

    Involve patients and carers in the planning anddelivery of care.

    Encourage staff to Value the carer/familycontribution and encourage a partnership of care approach.

    Be the patients advocate and ambassador respectingpatient confidentiality and privacy with respect for diverse culturalbackgrounds and requirements.

    Leadthe development of a patient focussed education and information giving strategywithin the speciality.

    Recognise,prevent, and diffuse potential areas of conflict and take a key role in themanagement of the outcome.

    Clinical Governance / Quality

    Ensure compliance with policies, procedures andclinical guidelines for self and others.

    Develop and influence policy making and clinicalguidelines internally and externally, necessary to support children and youngpeople accessing the service.

    Continually monitor standards of care and leadthe improvement of care through benchmarking, Nursing Charter Audits andClinical Audit.

    Support and facilitate research ensureevidence-based practice in the service.

    Disseminate good practice and knowledge bypursuing opportunities to present work at national conferences or via publicationand encourage team members to do likewise.

    Promote patient and public involvementactivities in the service leading to service improvement.

    Promote peoples equality, diversity, and rights.

    Identify clinical issues and incidents withinthe Trust that reduce the quality of care within the service brining any issuesto the attention of the Head of Complex Needs or mental health champion CYP.

    Take part in activities to counter these toensure the delivery of safe and effective care.

    Participates and takes the lead in clinicalaudit within the service supporting others to participate and ensuring agreedresults are implemented.

    Education / Training

    Takes responsibility for own continuing professionaldevelopment and performance, including identifying own development needs andmaintain own portfolio in accordance with re-registration requirements.

    Participates in the supervision and the teachingof pre and post registration students, new staff members and junior medicalstaff.

    To lead and develop with the Head of ComplexNeeds and mental health champion CYP on educational programmes in relation tothe speciality.

    Supports the clinical area in ensuring theclinical environment is conducive to effective learning.

    Identifies training needs and develop trainingresources for nursing staff to enable role development and to meet the needs ofthe service.

    Take responsibility for providing complex needscore competencies for untrained and trained staff.

  • Experience of working autonomously within the specialist area at an advanced level delivering effective patient focused care.
