    Job summary

    This is an exciting opportunity for a self-motivated, flexible and innovative nurse. You will be committed to providing high quality care to contribute to the provision of specialist evidence based Cardiac Rehabilitation care within the hospital inpatient and community Cardiac Rehabilitation setting in Swindon. This includes the delivery of digital Cardiac Rehabilitation and face to face Cardiac rehabilitation sessions.

    You will have recent experience of working within the cardiology or Cardiac Rehabilitation setting.

    You will be required to demonstrate excellent communication skills and the ability to work autonomously as well as part of a multidisciplinary team.

    You must be willing to work 2 day shifts 08:30-16:30 hours ( Tuesday and Thursday)

    Main duties of the job

    The post holder will be responsible in collaboration with other members of the multidisciplinary cardiac rehabilitation team to plan, deliver and evaluate all elements of evidence based comprehensive Cardiac Rehabilitation to patients following acute cardiac events. They will be responsible for his/her own workload and will be expected to provide local clinical expertise and knowledge in Cardiac Rehabilitation, in line with National guidelines thereby promoting best practice.

    Job responsibilities

    Use specialist skills and knowledge to assess individual patients referred to the Cardiac Rehabilitation service.Provide patients and their families appropriate and evidence-based advice regarding lifestyle modification, cardiac event and recovery.Accurate and legible documentation should be completed to record Cardiac Rehabilitation assessments/interventions.Contribute to a patients seamless care between secondary and primary care by providing good written communication to the patients GP and patient regarding individual assessments and behaviour changes/goals agreed.Signposting and referring where appropriate patients to other health professionals or supportive services , smoking cessation, dietetic team, psychology and update patient records on the National audit of Cardiac Rehabilitation (NACR) database

    Person Specification

    Education

    Essential

  • NMC Registered nurse (level 1)
  • Diploma/Degree in nursing or willingness to undertake study at this level.
  • Basic life support with AED training
  • Desirable

  • Relevant post graduate course in cardiology, cardiothoracic nursing or coronary care.
  • Level 2 smoking cessation training
  • Heart Manual facilitator
  • ILS/ALS or willingness to undertake
  • Teaching Qualification
  • Experience

    Essential

  • Significant experience of working within a Cardiology or Cardiac rehabilitation setting.
  • Significant post Registration nursing experience
  • Desirable

  • Experience within a health promotion role
  • Knowledge & Skills

    Essential

  • A good standard of written and spoken English is required to be able to undertake the relevant duties.
  • Basic IT Skills including use of email, inputting data, power point
  • The ability to dictate clear concise letters
  • Ability to prepare and deliver presentations to groups of people
  • Ability to communicate complex information to patients and relatives who may be anxious
  • Desirable
