- Matching the client with the appropriate practitioner, including contacting the practitioner for their availability
- Liaise with GPs, solicitors, employers, occupational health and medical insurance companies regarding referrals when applicable
- Refer any 'at risk' cases to the appropriate clinical manager or safeguarding member of staff
- Deal with enquiries from clients and customers
- Good understanding and knowledge of potential major adaptations at resident's properties
- Clear understanding of what DFG's are and how these are being provided to resident with long term physical needs
- Ability to engage with vulnerable clients and work collaboratively with colleagues, be a team player with a can-do attitude and a positive outlook
- Able to manage own time well and prioritise to meet deadlines
- Hold a Full UK Drivers Licence to meet travel requirements for the role

Part of The Norse Group, N-Able specialise in providing mobility equipment and assistive technology to customers and clients across the County. Our aim to keep customers safe, happy and healthy in your own home for as long as possible.
Due to recent expansion, we currently have an opportunity for an Occupational Therapy Case Worker to join our team in Buxton, working as part of our new joint venture partnership with High Peak Borough Council and Staffordshire Moorlands District Council.
The Role
You will play a key part in our Case Management Team, being the first port of call when customers contact us to access and discuss our services. This front facing position which requires you to provide a complete customer service experience, gathering understand the caller's needs and explain how our Disabled Facilities Grants (DFGs) services work and coordinate the client with the appropriate practitioner.
