    Stockport NHS Foundation Trust have had a new investment of funding to expand their established Integrated Community Neuro Rehabilitation Service (CNRS).

    We are looking for an enthusiastic, innovative, experienced Assistant Practitioner to join our friendly, forward-thinking team providing rehabilitation for a diverse range of people with stroke and neurological conditions within Stockport.

    Main duties of the job

    Based in the community, you will work within a comprehensive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Psychologists, Nurses, Therapy Assistants and Admin. You will be welcomed as a valued member of the coordinated and committed team, sharing knowledge, skills and opportunities to learn.

    You will help to facilitate the early supported discharge of stroke patients from the stroke unit, and work with patients with acute and long term neurological conditions to provide rehabilitation in the home environment, and other community settings, improving their quality of life.

    Job responsibilities

    Support and close collaboration is provided with the opportunity to work directly with experienced and specialist clinicians across the team, and you will be given the opportunity to provide support to band 3 Therapy Assistants within the team.

    Joining the Community Neuro Rehab Service (CNRS) in Stockport, you will see were a supportive, caring and proactive team. Everything we do is aimed at providing exceptional, holistic, person-centred care to people with stroke and neurological conditions within the community of Stockport.

    Education & Qualifications

    Essential

  • Foundation Degree in Health and Social Care (or equivalent qualification or equivalent experience in relevant clinical area )
  • GCSEs (Grade C or above) in Maths and English Language (or New GCSE Grade 4-9) OR Functional Skills Level 2 in Maths and English or Equivalent (not entry level 2)
  • Evidence of Continuing Professional Development
  • Desirable

  • ECDL or equivalent IT qualification
  • Knowledge

    Essential

  • Working Knowledge of physical disability
  • Working knowledge of relevant legislation
  • Working knowledge of relevant therapy interventions appropriate to clinical area
