To undertake specific skilled support work to aid the efficient and effective delivery of Stroke Rehabilitation to patients with stroke disease within the framework of clinical governance To work as part of the community stroke team providing high quality, effective and efficient rehabilitation to patients with stroke disease To accept patients triaged or referred by a physiotherapist, occupational therapist or speech and language therapist and be responsible for their own patient caseload.

Work unsupervised in patients own homes, to assess, treat, progress and discharge patients aligned to protocol, and inform therapists of any problems.

To provide personalised self-condition management programmes.


To coordinate, supervise and teach junior therapy assistants To adhere to all Trust policies and procedures as they pertain to personnel, health & safety and safeguarding needs.

To participate in continuing professional development under the guidance of a therapist.

To maintain communication with the patient, the carer and associated professional practitioners, and refer onward to other agencies as needed.

To maintain up to date electronic medical records and document clinical outcomes.
