Description
This post is based at Simmons House, a twelve-place in-patient unit for adolescents years) with severe and complex mental health needs.

The post holder will work alongside the Lead and Band 5 OT to provide meaningful structured activities for the young people at Simmons house in the afternoon and early evenings as well as full days during the school holidays.

The post holder will also work alongside the nursing team and take a lead on facilitating structured activities.

  • To provide interventions with recovery principles to improve the young peoples' quality of life and to support them to do the things they want to do and live the life they want to lead.
  • To support with facilitating a therapeutic programme includes social, creative and skillsbased activities which promotes social inclusion and encourages meaningful occupation for the young people in the service.
  • To be responsible for planning, implementing, and evaluating group under the guidance of the lead occupational therapist.
  • To work closely with the Multi Disciplinary Team (MDT) in leading on the facilitation of client centred activities in the evening

Clinical

1.1 To provide culturally appropriate activities for the young people.


1.2 To work as a team member to deliver clinical interventions under supervision and direction, using activities as a treatment medium to young people to achieve / develop/maintain daily living skills.

1.3 To support young people to engage in social, diversional and recreational activities.


1.4 To plan, implement and review a range of activities using individual and group work approaches using clinical reasoning, under the guidance and direction of qualified of the Lead OT.


1.5 To support young people to engage in care plans which have been developed in partnership with the individual and that reflect their goals.


1.6 To support and assist young people with managing their self-care needs as and if identified as part of a care plan under guidance of the OT.


1.7 To assess risk and manage it effectively whilst ensuring maximum 'risk tolerance' that facilitates the individual's recovery, in line with risk policies and local guidelines.

1.8 To work in partnership with other agencies, for example signposting to external community leisure resources.


1.9 To be aware of and sensitive to the safeguarding needs of children and adults, and knowledgeable both about potential indicators of abuse or neglect and about what to do if they have concerns about a child's or adult's welfare.

1.10 To provide information regarding the delivery of activities to all service users and other professionals within the team.

1.11 To offer regular monitoring of mental state.


1.12 To be aware of community resources in order that a socially inclusive focus towards intervention and discharge planning is maintained.


1.13 To escort service users to various therapeutic / treatment programmes as determined by their care plan within their community setting.

Communication


2.1 To demonstrate effective communication skills and form positive working relationship, using warmth and empathy that enables service user empowerment.

2.2 To demonstrate skills and resourcefulness in communication when barriers to understanding are present, including utilising interpreting resources.

2.3 To promote the role of an activity coordinator within the service.

2.4 To maintain partnership working with the other clinical service settings, and other statutory and non-statutory providers.


2.4 To handover information to wider MDT and lead OT about young peoples engagement /presentation in group programme or individual sessions.

Documentation

  • To ensure timely, accurate and informative records of clinical contacts are recorded meet all standards, according to Trust policy and CQUIN targets.
  • To contribute to the recording of service users intervention plans under the supervision of the lead occupational therapist.
  • To accurately document young peoples progress in the report format used in the service and be open to amend or evaluate reports if young people would like their contribution added.
