Our aim is to improve care for patients by working collaboratively across primary care and our partners as part of the Integrated Care Partnership.

The Confederation team also work to develop and support individual GP practices, PCNs and Neighbourhoods and their changing needs. We are of the NHS but independent, innovative and transformational.
General capacity across primary care is being expanded rapidly.

Job Summary:

This role will provide administrative support to the Transformation Team and the Primary Care Networks.

The post holder will help with the development and execution of administrative support and process, participate in the identifying and solving of logístical issues and collaborate with multiple stakeholders on a day to day basis.


Primary Responsibilities:

Work as a key member of the Transformation Team providing:

  • Collation and continuous update of the teams project dossier to ensure all programmes of work are captured
  • Working with all team members in the collection of information for performance reporting on relevant team projects.
  • Support with the minute taking of key meetings include the Neighbourhood Steering Board
  • Undertake diary management duties including arranging and changing appointments, prioritising these as appropriate.
  • Inputting, monitoring and checking data, required for finite and ongoing projects within the teams
  • Support the 6 Hillingdon PCN Clinical Directors and their leadership teams to provide robust secretarial and administrative support aimed at helping these busy clinicians achieve both national and their local objectives. This would include;
  • Supporting PCN leadership team meetings collating and circulating papers, taking minutes, following up on actions.
  • Being point of contact for PCN Clinical Directors, dealing with and directing queries and communications in the most appropriate and effective way.
  • Aiding the design and maintenance of accurate databases and records where these are needed.
  • Maintaining effective, up to date secure and appropriately accessible record management systems as per GPDR and Data security requirements.
  • Assisting PCNs to set up and maintain accurate communication streams, ensuring the right people have the necessary information at the right time. Disseminating communications and messages as appropriate.
  • Supporting the development, implementation and maintenance of best practice administrative processes and systems. Templating and sharing these with fellow PCNs and colleagues.

Collaborative Working Relationships:


  • Foster and maintain strong links within the Confederation and across the PCN and partner organisations.
  • Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.
  • Demonstrates use of appropriate communication to gain the cooperation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. CCGs).
  • Can recognise personal limitations and refer to more appropriate colleague(s) when necessary.
Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to:

  • Patients and their representatives.
  • GP, nurses and other practice staff.
  • Other healthcare professionals including community pharmacists, pharmacy technicians, social prescribers, first contact physiotherapists, physicians associate and paramedics.
  • Community pharmacists and support staff
  • Other members of the medicines management (MM) team including pharmacists, technicians and dieticians
  • Locality/GP prescribing lead
  • Community nurses and other allied health professionals
  • Hospital staff with responsibilities for prescribing and medicines optimization
  • Community nurses and other allied health professionals

Professional Development:


  • Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
  • Involved in one to one meetings with line manager monthly to discuss targets and outcomes achieved.
  • Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
  • Review yearly progress and develop clear plans to achieve results within priorities set by others.

Knowledge, Skills and Experience Required:
