Job Description:
Are you an experienced social care professional with a passion for supporting young adults with disabilities during their transition to adulthood? Harrow Council is seeking a dedicated Senior Practitioner to join our CYADS (Children & Young Adults Disabilities Services Transition Team.

As a Senior Practitioner, you will play a pivotal role in providing guidance, support, and management oversight within our dynamic team.

Key Responsibilities:

Provide expert advice and support to social workers and care managers on casework, ensuring compliance with the Care Act 2014 and Children's Act 1989.

Manage duty responsibilities, including chairing Professional Meetings/Case Conferences and ensuring timely response to incoming concerns.
Conduct complex enquiries, collaborating with internal and external stakeholders as necessary.
Supervise and mentor up to four team members, fostering a culture of excellence and ethical practice.
Collaborate with other internal services such as Enablement, Shared Lives, and SEN to deliver holistic support to service users.
Participate in milestone-based approaches for young adults with EHCPs, including attending important annual reviews and school/college events.
Assess training needs within the team and facilitate relevant training sessions to enhance service delivery.
Maintain accurate records on the Mosaic database and ensure thorough, cost-effective decision-making in all casework

Qualifications and Skills:
Demonstrated experience in social care practice, with a strong understanding of relevant legislation and policies.
Excellent communication and interpersonal skills, with the ability to collaborate effectively with multidisciplinary teams.
Proven leadership abilities, including the supervision and development of staff.
Sound judgment and decision-making skills, with a commitment to person-centered approaches.
Willingness to work occasional unsocial hours and conduct visits in the community as required
