- RN Child/RSCN or equivalent

Description
The post holder will be responsible for the assessment, planning and delivery of evidence based nursing care in the community setting to a caseload of children/young people aged 0-18 years.
The children on our caseload have a variety of chronic and acute conditions requiring nursing intervention and support such as enteral feeding, long term ventilation, CVAD care, and complex symptom management including palliative and end of life care.
Use of enhanced communication skills to promote good working relationships are required in order to facilitate the best outcomes possible for the child/young person. Job description Job responsibilities Working within the multidisciplinary team and across partner agencies to develop good relationships with the families, children and young people to maintain high standards of care at all times.
To provide holistic care to the child and family ensuring that timely referrals are made to other agencies as necessary.
To ensure professional skilled nursing care is provided to patients within the community/home setting and adapted to meet individual needs.
To provide clinical expertise by setting standards in the assessment of patients careneeds using agreed protocols within the scope of the service.
To provide clinical expertise in providing education and support for the children andfamilies in the community setting.
To provide holistic care to the child and family ensuring that timely referrals are made toother agencies as necessary.
You will deputise for the Lead Nurse in her absence, regularly provide management in the day to day running of the service and provide supervision to junior members of the team and mentorship to students in training.
Our core service hours are Monday Friday, working contracted hours as a shift pattern of early shifts and late shifts across the working week
The service supports children and their families during end-of-life care and flexibility of work practice to be able to support these times is necessary- working flexibly across a 7 day service, including unsocial hours and on call on occasion when required.
