JOB PURPOSE The main purpose of this role is to lead the Integrated Transfer of Care Hub and the services that sit within the hub, managing the services as one, integrated team, sitting under a single management structure.

This includes the line management of the leads for the services that sit within the Transfer of Care Hub.

The post holder will provide daily oversight on system flow, including capacity and demand, and to highlight to Alliance Leaders areas that require escalation.

The Integrated Transfer of Care Hub Lead will have accountability for the operational and strategic direction for timely discharges from acute settings to health and social care providers in the community.

The post holder will provide expertise in the performance and management of the Integrated Transfer of Care Hub.

Working closely with the system-wide Health and Social Care Senior Operational Leads, the post holder will work with respective leads in these areas to ensure flow across the pathways.

The post holder will lead the operational element; expedite discharge times, minimising barriers and improving patient experience across all discharges (Pathway 1-3) within the system, escalating to senior leaders as required.

The post holder will monitor, review and report performance in relation to discharge and delays for Pathways 1-3 within the system.


The post holder will work with system leads to develop and implement robust systems to ensure consistent and collaborative working with partner organisations.

Working with key stakeholders, the post holder, will lead evaluation and continuous improvement of the service to support the delivery of improved discharge pathways and the delivery of regional and locally agreed improvements in response to the Hospital Discharge Guidance March 2020 and to minimise the number of patients with no reason to reside in an acute hospital bed.

The post holder will work as an Alliance Lead for the operational delivery of system wide agreed discharge standards and related policies.


KEY TASKS:
Key Responsibilities a) Provide leadership to the Integrated Transfer of Care Hub, providing a focus on system flow promoting Home First principles to facilitate timely transfers of care for patients with no reason to reside.

e) To ensure continuous improvement with flow and capacity, managing changing demands across the year, including supporting the business planning activity, and advising Alliance Leaders on the impact on capacity and flow of service developments and changes. f) To maintain oversight of national, regional, and local policies, relating to transfers of care for patients without reason to reside and ensure that these are understood and reflected in local system policies and procedures.

g) To plan and maintain an overview of Surge and Escalation policies and procedures arising from the local Operational Delivery Group and maintain a standing brief to the Alliance Delivery Lead (Social Care) and system leaders of any changes or risks or issues of significance relating to system flow to the Alliance.

h) To act as the point of co-ordination between the Integrated Transfer of Care Hub, Alliance partners, tactical, contracts and informatics, specifically with respect to data flow.

i) Provide analysis of performance data and comparison of targets with actual performance and interpretation of the reasons for divergence to meet demand, producing reports for senior leads and Alliance partners as required. j) Developing areas of good practice with other ICS Transfer of Care Leads. k) To lead on routine escalation calls, ensuring that agreed actions are implemented and maintaining daily briefings to the Operational leads across the system. l) Support to the management of overall finances for the service and ensure robust budget management is place throughout the teams with appropriate controls in place to ensure spending profiles are met whilst saving challenges are delivered.
