    Job summary

    We are looking for an enthusiastic Care Co-ordnator to join our partnership

    You will work within our lovely nursing team that work across both our Bakewell and Tideswell surgeries. Our surgeries have around 9,000 patients and are both set in lovely rural locations.

    The applicant will work closely with our Community Matron maintaining a caseload, completing care plans with guidance from the clinical team, liaise with outside services, attend the weekly meeting.

    Main duties of the job

    Be friendly and approachable, work well as part of a team, but also be able to work alone.

    You will have compassion and empathy

    Be able to undertake home visits

    Be willing to learn new skills in a quickly changing environment

    About us

    We are a two partner Practice based over two sites. We have some patients that require a bit more support and are under the care of our Community Matron.

    The role will be to provide admin support for the patients and for the Matron.

    Job responsibilities

    Working With the Support of the Community Matron and Nursing Team you will assist the practiceclinical team in the provision and delivery of prescribed programmes of patientcare.

    Duties and responsibilities

    Assistingin the assessment and surveillance of patients health and well-being

    Helpingto raise awareness of health and well-being and how it can be promoted

    Assisting with the collection andcollation of data on needs related to health and well-being

    Attending clinical meetings and assisting with plans

    Completing care plans and liaising with other agencies
