    Job summary

    PRACTICE NURSE - 18 hours per week

    Looking to work in a well organised, happy and supportive environment ?

    We are looking for a friendly and highly experienced Practice Nurse to join our team. The successful candidate is someone who is motivated and committed to providing high quality care, working collaboratively with the general practice team to meet the needs of our patients.

    Main duties of the job

    Long term condition management

    Family planning & woman's health including cervical smears

    Travel health advice and vaccinations

    Child immunisations and advice

    Men's health screening

    ICP

    QOF Planning & Implementation

    DES & LES Implementation

    Padgate Medical Centre is well established, purpose built GP surgery located tothe East of Warrington serving approximately 6800 patients.

    Job responsibilities

    Job Summary

    To provide and maintain a high standard of nursing care for patients, as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and any additional roles agreed between the nurse and the doctors as appropriate, having regard to current training.

    Job Responsibilities

    Professional

  • Able to document Consultations according to NMC guidelines.
  • Ensure awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse.
  • Offer an holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection.
  • Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
  • Run well person clinics and health promotion programmes, facilitating change by providing holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise.
  • Assess problems presented opportunistically by patients, dealing with minor illness.
  • Perform an holistic assessment of patients attending for cervical cytology smear tests.
  • Perform venepuncture according to local guidelines.
  • Take ECGs.
  • Recognize and manage anaphylaxis according to current UK guidelines.
  • Perform Cardio-pulmonary resuscitation according to current UK guidelines.
  • Assist in the provision of minor surgery (only for sickness/annual leave cover).
  • Obtain and document informed consent (either verbal or written).
  • Ensure infection control guidelines are maintained
  • Monitor and manage maintenance of stock and equipment to include refrigeration, sterilizer and emergency equipment

  • Knowledge and understanding of policy developments related to the delivery of primary care services. Familiarity of the health needs of the Practice population. Understanding evidence based practice. Knowledge of national standards that inform practice ( National Service Frameworks, NICE guidelines etc.) Accountability of own role in line with NMC Code of Professional Conduct and medico-legal aspects of the Practice Nurse role.     Essential

  • Recent and relevant experience of working in Practice Nurse role within Primary Care. Understanding and experience of long term condition management. Maintenance of up-to-date skills, knowledge and on-going competence of the Practice Nurse role. Evidence of capacity to work successfully, both as an autonomous clinician and as part of a team. Proven ability to evaluate the safety and effectiveness of own practice. Interpreting and implementing local and national policies.

  • Ability to provide good, quality care to enable positive patient outcomes. Clinical examination. Good interpersonal and communication skills - both written and verbal. Experience in using IT in Primary Care setting. Good team player.
