Hampshire County Council's Care Leavers' teams are committed to improving outcomes for young people when they leave our care. Working with young people aged 16-25 in their transition to adulthood and independent living, you will have considerable experience of working with young people, and of liaison with other agencies in putting together support plans.

You will demonstrate the ability to build relationships with young people, face difficult issues and deal with them sensitively, and to be a team player.
