Clinical Role To work within agreed local processes to make informed decisions regarding Continuing Healthcare eligibility based upon initial health needs documentation provided by health and social care staff.

To ensure that those individuals in receipt of Continuing Healthcare funding and NHS Funded Nursing Care are regularly reviewed and monitored to ensure that the levels of funding are appropriate, and the client is cared for appropriately.

To demonstrate highly developed specialist knowledge within Continuing Healthcare and NHS Funded Care acting as a reference for colleagues and other agencies.

To act as a positive role model at all times through the promotion of visible leadership To act as a resource for all members of the multi-disciplinary team, providing expert nursing advice concerning aspects of patient management.

To ensure that re-evaluating the rationale for placement, appropriateness and funding is taking place, for all patients.

To be responsible for monitoring the standard of assessments, planning, implementation and evaluation of care ensuring the quality of care provided is of a high standard.

To promote evidence-based practice utilising measurable standards and outcomes to demonstrate and improve the quality of care. To develop a comprehensive service that is flexible and innovative meeting the needs of the client group.

To make recommendations for service improvement and to lead on the developments of new initiatives.

Continually develop own nursing expertise, knowledge, and skills to enhance effective care and support and to ensure this expertise is shared with other clinical staff.

To champion the development of leadership competencies within the team.

To assist in the development, implementation and audit of guidelines, protocols, and pathways to ensure that each patient/client has an effective management plan.

To lead on user involvement in the planning and evaluation of service provision. Lead the process of integrated audit and other supporting audits to monitor.

To facilitate an environment where informed patient/client choice and quality measures are developed and utilised as part of standard work.

To support Continuing Healthcare Strategic Lead to have a vision for the service area to meet the demand.


To ensure that the Band 6 staff have up to date PDPs and appraisals and appropriate training and development opportunities.

To manage the sickness absence of the Band 6 staff. To act up for the Continuing Healthcare Strategic Lead as appropriate.

To ensure effective communication systems are established and maintained both within and across the team and to act as a key link in facilitating good communication through attendance at specific meetings.

To facilitate the development of self-awareness of self and others in the team through reflection. To maintain accurate records and statistical data on workload, caseloads and work delivered.

To be proactive and contribute in planning and strategic development through the development and implementation of the service improvement plans.

To create and maintain a culture which reflects the principles within improving working lives.


To ensure that staff are aware of policies and procedures and are working within the boundaries and scope of such.

To take responsibility for regular performance review of staff and facilitate and monitor performance accordingly. To be actively involved in the management of poor performance of any member of staff within the team. To ensure any issues raised by patients/clients and families are proactively managed.

To advise, support, maintain and improve the health, safety and security of staff in the workplace. To ensure that all staff are aware of their responsibilities in relation to health and safety. To ensure compliance with Health and Safety legislation including COSHH, Workplace risk assessment and Control of Infection Policy. To complete all risk assessments, records, and audits to demonstrate compliance to health and safety requirements.

To implement the outcomes and recommendations from SABs alerts, NPSA alerts and any other safety bulletin.

To work within and maintain practice according to NMC guidance and ICB policies To be responsible for the recruitment and selection of nursing staff, ensuring induction programmes are in place and undertaken as appropriate.

Communication To provide comprehensive timely feedback to relevant others i.e., professionals, patients, and carers regarding outcome of Continuing Healthcare consideration/ assessments.

To ensure that information is cascaded to the team as appropriate e.g.

Chief Executives Bulletins, ICB Communications. To ensure that safeguarding alerts are communicated as appropriate. To complete risk notification forms when concerns in care are identified. Documentation To complete comprehensive assessment documentation showing rationale to justify decision making.

To prepare reports as requested by the Continuing Healthcare Strategic Lead. To
