Description
Provide telephone advice and refer on or signpost to other sources of support.
  • Communicate and signpost to appropriate needs related information.
  • Guide people through the use of self assessment resources.
  • To utilise verbal and non verbal communication skills to effectively communicate with patients, their families and carers.


This may include patients/carers who may or may not have barriers to communication for example, language through cognitive or physical impairment.


  • Deliver Personal Care and Support Assessments and the development of a Personalised Care and Support Plan
  • Implement, monitor and review the Care Plan with the patient and carer, in line with standard operating procedures and protocols and modify as appropriate.
  • Evaluate outcomes of care delivery as appropriate.
  • Identify indicators of need or changes in need through telephone contact and respond appropriately.
