    Community CNS TEAM - Clinical Nurse Specialist Palliative Care,Band 7, Full time37.5 hours per week

    Inner North West Division - Pembridge Specialist Palliative Care Service.

    The Pembridge team practice holistic specialist palliative care taking a multidimensional approach to physical, emotional, social and spiritual needs of their patients and their families. The service comprises of a community team, day services, outpatients and inpatient unit ( ward currently suspended to admissions awaiting the outcome of the NWL Palliative Care Review)

    An exciting opportunity has arisen for a clinical nurse specialist to join our community team. We are looking to recruit a highly motivated and compassionate registered nurse with the skills and knowledge to fulfil this role.

    The successful candidate will be expected to demonstrate a high level of expertise clinically, have excellent communication skills and the ability to work flexibly with consideration to the needs of the service. This post participates in the CNS weekend working rotation.

    The role is based at the Pembridge Hospice, St Charles Centre for health and Wellbeing.

    Main duties of the job
    • To provide a specialist nursing service for patients and their families/ carers with specialist care needs
    • To liaise closely with colleagues at the Pembridge Hospice, other specialist services as well as primary and social care to facilitate continuity of care for patients.
    • To offer support, advice and education to the primary health care tea, hospice, and hospital staff
    • To participate in quality initiatives for service improvement.
    • To champion palliative and end of life care across the Trust
    Key Working Relationships

    Pembridge community palliative care team, the Hospice inpatient and day unit and the wider multidisciplinary team.     • To accept responsibility, as the named community palliative care keyworker for own patient caseload
    • To provide expert symptom control advice, emotional, and spiritual care and support to patients and their families/ carers
    • To independently assess patients and carers needs, identifying their goals and work with them towards achieving them.
    • To ensure patients have an individual plan of care which reflects the assessment undertaken and incorporates issues and recommendations made.
    • To recognise and manage palliative care emergencies appropriately.

    • First level degree in related subject (or evidence of working towards this

    • Teaching and Assessment qualification
