    Clinical Pharmacist

    £50,000-£60,000

    Are you a skilled pharmacist with a passion for patient care and improving medicines management? If so, we have an exciting opportunity for you to join a friendly and growing GP Surgery team as a GP Practice Pharmacist.


    Working as part of a multidisciplinary team within a GP practice, you will provide patient-facing dispensing, advice, treatment and administrative support.

    As a clinical leader for medicines optimisation, you will manage the prescription process and queries, undertake clinical medical reviews, and support patients with chronic disease management issues.

    You will also assist with communication across a patient's care pathway, manage medicines shortages, and mentor newer pharmacists.


    Key responsibilities:

    • Providing clinical expertise and being an educational resource for colleagues in relation to medicines
    • Liaising with and developing relationships with a multidisciplinary team
    • Managing the process of medicines reconciliation on admittance or following discharge from hospital
    • Managing, auditing, and reviewing prescription policies to improve the quality, safety, and cost-effectiveness of prescribing
    • Reducing wasteful polypharmacy and inappropriate and wasteful prescribing of medicines
    • Proactively seeking to reduce medicine-related hospital admissions and readmissions
    • Handling patients and healthcare professionals' prescription queries
    • Researching and identifying patients that require medication reviews
    • Performing regular patient reviews to identify and address any medicines-related issues
    • Making appropriate recommendations to GP and other healthcare professionals
    • Ensuring repeat reauthorisation processes are adhered to for high-risk medicines
    • Undertaking and dealing with minor ailments and triaging of patients.

    Qualifications:

    • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
    • Minimum of 2 years post-graduate experience in pharmacy
    • Member of the Royal Pharmaceutical Society (RPS)
    • Member of or working towards (through foundation) RPS Faculty membership
    • Hold or be working towards a prescribing qualification
    • Hold or be working towards a postgraduate pharmacy qualification
    This is a full-time permanent position working from Monday to Friday. There is a Saturday pharmacy rota (which can be done remotely) and is four hours every three weeks.
