Hours: 37.5 hours per week
  • Monday to Friday
Opportunity for overtime payment covering oncall on a rota basis

Main Responsibilities of a Care Coordinator

  • Assist with the implementation and set up of new packages, working closely with all involved in this process
  • Oversee all aspects of clients care packages (updating care plans and risk assessments, completing monthly reviews etc).
  • Build and maintain professional relationships with clients and carers based on trust, respect and honesty.
  • Assist with delivering effective staff inductions including meet and greets and shadow shifts.
  • Complete care staff supervisions, appraisals and team meetings.
  • Ensure all shifts are filled and plan in advance for any annual leave.
  • Report any incidents/accidents or concerns in a timely manner through our internal recording system.
  • Participate in the out of hours on call rota (additional pay).
  • Complete any other appropriate duties and responsibilities as requested by the Registered Manager

Experience/Skills Required for this Care Coordinator role:
