    Job summary

    We have an exciting opportunity for a practice nurse to join our team. Ideally, we are looking for an experienced practice nurse, who can work autonomously as well as being a team player and someone who can support the delivery of policies and procedures within the practice. However, for the right candidate, training and support could be given to a general nurse to become a practice nurse.

    Main duties of the job

    Administer adult and child immunisations and vaccinations in accordance with national and local programmes. Ensure safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.

    Management of chronic diseases which includes asthma, Diabetes, hypertension, COPD, CVA/TIA, CHD, and CHD risk assessment and management.

    Provide focus for nursing expertise within practice, advising nursing staff, appropriate to level of competence.

    Undertaking cervical smears.

    Initiates and carries our programmes of Health promotion and screening.

    Contraception and sexual health advise.

    Help and support with clinical audits including infection control.

    Partake in patient investigations such as spirometry, feno and ECGs.

    Assist clinical staff with examinations and procedures.

    We are a friendly, patient focused, team orientated practice situated Oldham, Greater Manchester serving 6600 patients. Our team consists of 4 GP partners, 2 salaried GPs, 1 practice nurse, 1 AP and 2 HCAs as well as a fully established admin team. Our practice is also a training practice with up to 9 trainees at different stages of training supported by the partners and salary GPs.

    Person Specification

    Experience

    Essential

  • Experience of chronic disease management
  • Ability to communicate effectively within the primary care team, including maintaining clear and effective records of patient contact.
  • The ability to empathise and reassure patients. Including breaking bad news.
  • Ability to work independently (within own scope of practice) and work effectively as part of a team.
  • To work within approved clinical guidelines/protocols, have an awareness of when to refer patients for further clinical assessment.
  • Efficient organisation skills
  • Good and effective time management needed for daily clinics.
  • Is competent in anaphylaxis and resuscitation techniques.
  • Practice independently in accordance with NMC code of conduct
  • Maintain accurate documentation of care and utilisation of IT
  • Desirable

  • Experience of working as a practice nurse or community nurse
  • Experience of using EMIS web /Docman
  • Qualifications

    Essential

  • RGN, current NMC registration
  • Full UK driving licences (expected for home visits)
  • Desirable

  • Chronic disease management qualifications in additional areas (asthma, COPD, CHD, diabetes)
  • Childhood Immunisation and Cytology qualifications or willing to train to achieve these qualifications.
  • Independent Nurse Prescriber
  • Spirometry/ Feno trained or willing to be trained.
