    Job summary

    Ward 1 is a 12 bedded General Medical ward which supports patient flow. The patients on Ward 1 are transitioning from acute care to their discharge destination. Teamwork and person-centred care are at the forefront of our working day.

    Main duties of the job

  • To demonstrate up to date knowledge of national and local professional issues and to lead/facilitate the implementation at a local level.
  • To undertake any other duties, required by the Line Manager appropriate to the grade and relevant to the post.
  • To promote collaborative working partnership and effective communication between all members of the multi-disciplinary team.
  • Promote health and wellness through practice and education of patients, relatives, carers and staff.

    Job responsibilities

    1. To assist in the maintenance of standards for professional practices in order to enhance the quality of care for To participate in the effective management of the unit To provide and participate in the functions required to fulfil the goals and services by the ward/ To assist in the setting and monitoring of standards of nursing care within the ward/ To encourage the participation and involvement of the patient and carer(s) in the assessment, management and evaluation of To liaise with all relevant disciplines involved with direct care and initiate health promotion and To work alongside multidisciplinary team in coordinating patient care and rehabilitation.

    8. To participate in quality improvements and audits on the ward.
