Housing Support Mentor based in
Bangor.


Housing Support Mentor


Hours:37.5 hours per week + sleep in nights


Salary:
£21,312.23 per annum

Hours will include weekends and bank holidays and unsocial hours (plus additional payments for worked unsocial hours: between 22:00 and 8:00 - 26% of hourly rate.

weekends 48 hours - 8.5% of hourly rate; Bank Holidays - 33% of hourly rate; Sleep-ins - £60.00 (only one of these enhancements is paid per any hour)


Holidays: 25 days per year plus 8 public holidays (pro rata)

Job Purpose:


To provide tenancy related support services for those individuals who have complex and co-occurring issues including, substance abuse and other addictions, mental health, housing and homelessness and offending behaviour and to help individuals move on to sustainable living.

Providing support that enables individuals to live independently and feel safe and secure with the long-term aim of them living substance free and independently in their own homes.

Working with individuals in areas such as employment, training and education, debt management and daily living skills to promote wellbeing and a healthy lifestyle, inclusive of partner agency working.

The housing service works to a shift pattern which includes unsocial hours and sleep-ins.


Main duties and Responsibilities:


  • Working with service users in a proactive time and task specific manner on a variety of issues to impact positively on issues relating to housing, employment, training, and education, offending behaviour and social isolation, substance misuse and mental health.
  • To provide a variety of support to service users identified as appropriate using an integrative personcentred approach consistent with the functional model and methods used by Adferiad Recovery
  • To establish a positive therapeutic relationship with all stakeholders to promote personal and social wellbeing.
  • Working with the referral panel to support the smooth transition for an individual into service including, assessment, risk assessment, support planning and reviewing of support in line with Adferiad Recovery case management.
  • To enable service users to administer their financial affairs as appropriate, working within FCA guidelines.
  • To participate in the provision of shift working on call, weekend and sleeping night as required.
  • To have commitment to and demonstrate an undertaking of equality of opportunity and its relevance to different sectors within the community.
  • Must demonstrate empathy towards Adferiad Recovery's beneficiaries.
  • To carry out all other tasks commensurate with the role.
  • Supporting and assisting occupants with the following:
  • Feeling secure within their home
  • Increasing independent living skills
  • Using appliances safely, undertaking routine and minor domestic repairs, and maintaining reasonable domestic standards
  • Personal finance, budgeting, and accessing benefits
  • Mental and physical healthcare
  • Selfmanagement of medication
  • Attending appointments
  • Building and maintaining relationships with neighbours, families, friends, professionals developing skills and interests
  • Pursuing training/educational/employment goals and aspirations
  • Pursuing leisure/recreational goals and interests
  • Developing links with the local communitybased resources and opportunities
  • Signposting, joint work and referral on to other specialist agencies, intervening in a crisis
  • Recognition of and coping strategies in respect of risk, triggers and avoiding relapse
  • Housing moveon planning and arrangements from the housing project (resettlement activities)
  • Promoting recovery
  • Data collection and appropriate recording and presentation for monitoring and Quality Assurance reports as directed by the Team Leader and Manager.
  • Maintaining records and statistics required to monitor and evaluate the effectiveness of the service.
  • Follow procedures for general record keeping and information collection, saving and archiving adhering to confidentiality and data protection policies.
