Description

We are the Islington Memory & Dementia Navigator Service, a high quality assessment, treatment and support service in the London Borough of Islington provided by the Camden & Islington NHS Foundation Trust, who are passionate about supporting people affected by dementia.

We have one of the highest diagnostic rates in the UK and have strong links with research at University College London.

We're seeking to recruit adementia navigator/assistant practitioner who is enthusiastic and motivated to help us deliver excellent dementia care in the community.

You will hold your own caseload and undertake reviews of people living with dementia, and their carers, living in their own homes.

You will be supported by a multidisciplinary team and work closely with the lead nurse for your catchment area.

As our model aims to support all residents living with dementia, we are a busy service and caseloads are reflective of this.
