    Paediatric Clinical Lead
    Thornbury Community Services (Part of Acacium Group)

    Field based with frequent travel across Hampshire and surrounding areas

    Permanent, Full Time

    £42,000-£45,000 + £3,000 car allowance + 5% annual bonus

    We have an exciting opportunity to join our thriving business as a Paediatric Clinical Lead.

    You will be responsible for overseeing the bespoke care and support of clients, helping them to achieve their full potential and experience an increased quality of life.

    You will assist the Senior Clinical Team in the assessment process and setting up new complex care packages, as well as carrying out patient centered care plans.

    Develop individualised care plans in partnership with the service user, family and other Health Care Professionals as appropriate ensuring goals are patient focused, realistic and achievable
    Undertake initial holistic nursing assessments in partnership with the Service User, family and carer and involving other statutory/voluntary/third 'party' sector providers as necessary
    Identify and manage risk as part of the care provision to clients whilst implementing precautions to safeguard the welfare and safety of staff, clients, visitors and the public, by implementing all policies related to health, safety, security and risk whenever applicable
    Effectively work as part of the wider field-based team, building a strong team-based approach to providing care

    A Paediatic Nursing degree with an active NMC pin
    Experience to band 6 or equivalent
    Nursing experience within the community
