    Job overview

    The post holder will lead on developing the metastatic breast pathway for the patients, relatives, and carers.

    Working collaboratively with the multidisciplinary teams within the breast unit and the oncology unit across the Trust.

    Liaising with team members to facilitate co-ordinate care that is patient centred for patients with secondary breast disease.

    Develop and hold nurse led clinics to provide a point of contact for patients with secondary disease either in person or by telephone contact.

    Work with the patients, families, and carers to use the holistic needs assessment tool to assess patient's needs, developing individual care plans during treatment and to re-evaluate as the ongoing care needs change.

    Main duties of the job

  • To lead on the development of the metastatic pathway for breast cancer patients, their relatives, and carers.
  • To work alongside the Oncologists and the multi-disciplinary team (MDT) in the diagnosis of metastatic breast cancer, supporting the patient, their relatives and carers throughout the cancer care pathway.
  • To assist in organising and planning complex case conferences, ensuring patient, carers and the correct range of professions and agencies are included.
  • To communicate and liaise effectively with other health and social care providers within the primary and secondary care setting to ensure effective management of the patients is maintained.
  • To develop and run metastatic breast cancer nurse led clinics to support patients, families, and carers.
  • Develop and participate in the on-going health and well-being support for patients living with and beyond cancer for example develop a metastatic cancer support group.

    Essential criteria

  • NMC registration
  • First level degree, or equivalent qualification
  • Demonstrates ability to work independently and as part of a team
  • Desirable criteria

  • Advanced communication skills course or similar
  • Demonstrates flexibility and ability to prioritise care
  • Demonstrates autologous practice
  • Experience and knowledge

    Essential criteria

  • Relevant clinical experience in Breast Cancer, Oncology and /or palliative care
  • Demonstrates knowledge of policies, practices and procedures e.g. NHS national policies, standards requirements and directions that relate to patient care
  • Continual Professional Development relevant to role
  • Desirable criteria

  • Band 7 experience
