    Job summary

    Working across the Primary Care Network comprisingof North Swindon Practice (employing practice) and Ashington House Surgery.

    Starting salary of £ per hour plus NHSPension

    Full-time permanent post, hours per week

    5 weeks annual leave plus 1 week study leave (prorata) plus Bank Holidays

    Training practice offering mentorship and support forprofessional development, which could include becoming an independentprescriber or gaining a postgraduate qualification in clinical pharmacy.

    Main duties of the job

    The successful candidate will work closely withour Senior Clinical Pharmacist and will potentially have a patient-facing role, workingdirectly with patients to promote safe and effective use of medication andself-care. The role will involve workingas part of a multi-disciplinary team to develop and run processes for medicinesmanagement on transfer of care and systems for safer prescribing

    North Swindon Practice (Home Ground Surgery and our branch surgery Crossroads) has one of the largest patient lists in Swindon, currently caring for over 20,000 people. In association with our Partner Practice in Brunel 3 PCN, we are looking for an additional Pharmacist to join our team of Pharmacists/Technicians who can help us develop medicines optimisation services within our Primary Care Network.

    Job responsibilities

    Thepost holder is a Pharmacist who works alongside and supports the general practiceteams within their professional boundaries. The role is pivotal to improving the quality of care and operationalefficiencies so requires motivation and passion to deliver excellent servicewithin general practice.

    Supportand mentorship will be offered by the practices to enable the Pharmacist to developin their role.

  • Masters degree in Pharmacy (MPharm) or equivalent Post graduate diploma or equivalent training/experience. Independent Prescriber (or interest in working towards this qualification).

  • KEY ASPECTS OF THE ROLE:
  • Work as part of a multi-disciplinary primary healthcare team in both a back office and patient-facing role.
  • Provide pro-active support for effective medicines management for patients suffering from chronic diseases.
  • Within the limits of professional competency manage medication for patients with common minor ailments.
  • Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Provide primary support to practice staff with regard to prescription and medication queries.
  • Support the practices repeat prescribing systems and deal with acute prescription requests and medicines reconciliation on transfer of care.
  • Implement systems for safer prescribing, providing expertise in clinical medicines advice.
  • Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
  • Ensure that the practices integrate with community and hospital pharmacies to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
