Job summary

The post holder is a member of the Neuro Rehab team/ Early supported discharge Stroke service and will be responsible for the assessments, planning, implementation, and evaluation of individualised physiotherapy therapy intervention to stroke and neurological patients living in the Bedfordshire community.

This will include range of community settings including patients home, care homes, rehab ward settings. This will include lone working within the community with access to additional therapist support when required.

Main duties of the job

To undertake comprehensive assessments, develops and implements specialised physiotherapy treatments/programmes for patients with stroke or a neurological diagnosis.

Develops specialist exercise programmes for patients to follow at home or when not with a physiotherapist. This may include teaching carers and caregivers in patient's own homes.

Works as part of multidisciplinary team including Occupational therapists, Speech & Language therapists, Psychologists and Assistant Practitioners in providing rehabilitation to Stroke and Neurological patients within the community.

Job responsibilities

Main Duties and Responsibilities

To undertake comprehensive assessments, develops and implements specialised physiotherapy treatments/programmes for patients with stroke or a neurological diagnosis. Develops specialist exercise programmes for patients to follow at home or when not with a physiotherapist. This may include teaching carers and caregivers in patients own home.

To use range of facts or situations as part of the assessment and provide appropriate physiotherapy from range of treatment options. To select and deliver evidence-based specialist physiotherapy treatments, agree goals with patients and monitor outcomes.

To involve patients and carers in the planning of rehabilitation programmes and encourage self-management where appropriate.

To select and administer highly developed physical skills, high degree of precision Dexterity, co-ordination, and sensory skills for assessment & manual treatment of patients. This will require sustained concentration.

Provide adaptive equipment, rehabilitation programmes, and make referrals on to other professionals and agencies as appropriate to meet the identified needs. To be responsible for all aspects of safety relating to equipment used. Occasionally, the post holder will have to pick up the prescribed equipments from the store.

To use developed clinical reasoning skills to review and evaluate patient progress and their current condition at each treatment session and to amend the treatment programme as appropriate to achieve agreed goals.

To plan and undertake the discharge of patients from Neuro rehab service when agreed goals have been reached or treatment is no longer appropriate ( the patient has reached a plateau or there is a change in their medical circumstances). To be able to negotiate this with patients who wish to continue treatment on an on-going basis.

To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.

To establish and maintain positive interpersonal relationships with other staff, clients, and their carers. Positive relationships are characterised by open communication, trust, and respect.

To take an active role in the stroke and Neuro rehabilitation duty process to prioritise and ensure timely response for assessments. To work flexibly across bases within Bedfordshire, as service needs require.

To have a good knowledge of supporting services available to clients from health, social care and voluntary agencies. To liaise with them to ensure planned care and intervention as agreed with clients is given.

To be professionally accountable for all aspects of own work including the management of patients in your care. To prioritise your own workload within the time available including the prioritising of patients to be treated along with other commitments in the day ( meetings). Senior staff will provide support, regular formal training, clinical reasoning sessions, clinical supervision and peer review for the post holder.

To understand and be acutely aware of significant medical observations during assessment and treatment of patients and report significant changes in patients conditions to the appropriate medical professional named nurse, GP etc.

To be responsible for coordinating intervention for designated clients acting as their case manager whilst they are receiving input from service. To organise and attend multidisciplinary case conferences and liaison meetings as necessary.

To work as an independent and autonomous professional. Act on own initiative, taking prompt and effective action in emergency situations. To escalate any concerns about staff and/or patient safety to more senior staff.

To be confident in their own diagnosis and be able to communicate that effectively to others (including medical professionals and other Allied Health Professionals).

To share information in a timely manner with more senior therapists if there are concerns about a patients safety, condition or progress.

To work a variety of shift patterns which may include weekends and bank holidays on a rota basis in the future. There may be occasions which are unpredictable, when the post holder may have to work late to support patients risk.

Communication

Using verbal, non-verbal and written communication tools, ensure that patients and carers have an understanding of their condition and the role and aims of physiotherapy in relation to this. The physiotherapist must be able to persuade, motivate and negotiate, as well as demonstrate empathy and reassurance.

To provide and receive complex sensitive information where motivational negotiating, empathetic and reassurance skills are needed, where barriers exist to understanding and the information could cause distress.

To form professional relationships with clients and communicate condition related information to them in a way that respects their views, autonomy and culture.

To communicate effectively with all members of the Bedfordshire Neuro Therapy Service and other providers to ensure optimum patient centred care.

The Physiotherapist must be able to adapt their communication, treatment, information and advice to meet the needs of individual patients/ families. Patients seen will range from the vulnerable ( elderly, mental health concerns or those with poor memory) to those who are extremely well informed about their condition.

The post holder to maintain accurate electronic patient records in a timely manner in accordance with HCPC and trust guidance.

Budgetary Responsibility

Personal duty of care in relation to equipment, resources/ handle cash, valuables; safe use of equipment other than equipment used personally; maintain stock control.

Responsible for equipment used in course of treatment/ handles patient valuables; responsible for physiotherapy equipment used by patients & others; orders supplies walking aids

People management

Responsible for day-to-day supervision and development of unregistered staff within Neuro and ESD service

To delegate tasks appropriately to Assistant Practitioners considering the complexity of the task and the skills and competence of that person. To oversee patient intervention and ensure that they deliver high quality patient care.

Provide support and education to new staff and students.

Research & Development Activity

To contribute to meetings about clinical governance and service development. To participate in delivery of the team development plan and also contribute towards policy development/proposal.

To participate in research to improve service as required

To continue to develop specialist Physiotherapy skills in a specific clinical field.

To be responsible for own competence to practice by on-going continuing professional development (CPD) and the recording of it in a reflective diary/portfolio. To maximise the learning opportunities available in order to support evidence based practice and influence clinical practice.

To exhibit and maintain a high personal standard of professional expertise, attitude and behaviour at all times in accordance with Chartered Society of Physiotherapy, HCPC and local Trust, and service policies.

Clinical and Practice Governance

Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

The post holder must adhere to the Trust risk assessment and risk management processes

The post holder must adhere to infection control policies and procedures

It is a condition of your employment that you are currently registered with HCPC and it is your responsibility to maintain your professional registration.

Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

The post holder must participate in clinical and other audits as required

Provide patients and their families /carers with information on standards they should expect from the team

Participate in clinical supervision on a regular basis. To be an active participant in the appraisal process. This includes prior preparation and being able to identify own learning needs and objectives.

The post holder is required to participate in relevant emergency preparedness process for their team.
