    Job summary

    An opportunity has arisen to join the Acute interventionteam (AIT). The AIT service supports the provision of care across the Truststhree acute sites by providing education and support for all healthcare staffcaring for the acutely ill deteriorating patient. Facilitating decision makingand communication between clinical staff, patients and relatives is a keycomponent of the role. The Team is firmly committed to ensure all patients aretreated by the right professional, in the right place, at the right time, firsttime, every time, 24 hours-a-day.

    This is an exciting role that willallow you to utilise your education and clinical skills to improve practice andensure that patients receive the best possible standard of care. The role wouldsuit any experienced nurse or paramedic with a background inacute/emergency/critical care who has an interest in teaching.

    1 x 30 hour fixed term temporary post until 13thof October and 1 x 30 hour fixed term temporary until 19th of January

    Main duties of the job

    To ensure that patients with clinical deterioration havetheir needs prioritised with appropriate assessment, intervention and on-goingmanagement. To ensure that patients atrisk are reviewed regularly and appropriate intervention initiated in a timelymanner.

    The post holder will assist with the delivery of educationthrough enhancing staff confidence, competence and experience of ward staffthrough sharing of skills.

    The service will utilise physiological track and triggertools to identify patients whose condition is deteriorating via EPR.

    They will assist/support audit projects and updateguidelines/policies within the Acute Intervention Nurse remit

    Assist / Support ward staff with care of the dying patients,complex DNACPR decisions and ensure effective communication with the patientand their families.

    To assist the anaesthetic team to stabilise critically illpatients throughout the hospital.

    To be an active member of emergency response teams withinthe hospital, taking a lead role in the Medical Emergency Team.

    Capable of acting as team leader in the Cardiac Arrest Teamwhen required.

    Active member of the Paediatric Emergency Team.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

    Effectively utilises members of the multi-professional teamwithin the Trust to enhance patient care and positively impact on patientsafety.

    Routinely work using own initiative and have the ability toseek advice when necessary from the most appropriate personnel.

    Provides continuing support and advice to ward staff whendealing with patients at risk of deterioration and those patients deterioratingin ward environments.

    Assist in the development and maintenance of policies forAcute Intervention Nurse, ensuring all policy and service developments areimpact assessed in respect of equality requirements.

    Assist as requested with recruitment, absent management,and training of staff.

    Encourage others to strive to develop excellence within theclinical area.

    Implements change processes which support Trust protocols,procedures and directives.

    Participates with audit projects.

    Keeps accurate and contemporaneous records of the patientscondition, treatment and progress as an on-going daily process.

    Updates the band 7 Senior Sister with all issues relatingto Acute Intervention Nurse activities within the Trust as an on-going process.

    Uses IT skills to produce management information, reports,audit data and other material.

    Attends team brief and other information giving meetings.

    Possesses skills of delegation, arbitration, negotiationand leadership.

    Performs audit of the Acute Intervention Nurse Service,evaluates findings and changes practice where required to improve patient orservice outcomes.

    Empowers and supports staff in the delivery ofcare to acutely ill patients.

  • Background in acute care area ED, AMU,ITU
  • Desirable

  • Experience in palliative care.
  • Special Skills & Knowledge

  • ILS provider. Experience of teaching junior colleagues in the clinical environment

  • ALS and / or EPLS provider. ALS or EPLS instructor or instructor candidate. Teaching qualification (998 or C&G) or relevant teaching experience. Ability to plan and prioritise workload. Able to make comprehensive patient assessment. Ability to make complex decisions. Demonstrate critical thinking. Experience in audit
