    An exciting opportunity has arisen to join the national Virtual Hospital Programme.

    This is an exciting transformation in how care is planned and delivered in England, building on the innovative Covid-19 Virtual Hospital work over the first two years of the pandemic.

    The Virtual Hospital provides specialist led care for a variety of patients including those with respiratory conditions COPD, Asthma Bronchiectasis, Heart failure, ARI, Frailty, and post operative colorectal patients but with anticipated expansion in acute medical conditions.

    Virtual Hospital care is provided to them in the comfort of their own homes using an advanced personal remote monitoring platform, enabling them to safely leave hospital sooner or not come into hospital at all.

    The virtual wards will have multi-professional input from consultants, specialist nurses a wealth of other health and social care staff. This is an exciting opportunity to develop new skills and help shape, develop, and expand clinical pathways that directly benefit patient care.

    We are currently recruiting into one Band 6 clinical nurse specialist post to provide care to virtual hospital patients.

    Main duties of the job

    As a member of the Virtual ward team, the role of Clinical Nurse Specialist will support patient care to be delivered in the home providing an alternative approach to receiving hospital care or clinical monitoring.

    A liaison points of contact for all patients providing signposting advice and guidance where appropriate.

    Communicate effectively and work in partnership with patients, their relatives/carers, nursing colleagues and members of the multi-disciplinary team to ensure care is delivered in an accurate and efficient manner.
