Discharge Specialist Practitioner (Sister / Charge Nurse / AHP
Therapist)


Band: 7


Base:
You may be required to work in other designated locations of the
change of base.


Reports to:
Matron


Accountable to:
Deputy Head of Nursing/ Head of Nursing (professionally
accountable to lead AHP therapist)

About the University Hospitals of Leicester NHS Trust: )

The post holder will

  • Be a registered nurse/ AHP with significant experience at band 6 or above, with experience within an acute hospital and /or community setting.
  • Trained to degree level with a teaching certificate or relevant experience.
  • Have excellent communication, negotiation and organisational skills and the ability to work within a multidisciplinary team.
  • Communicate and network at all levels within SM CMG and partners across the Primary Health and Social Care Community.
  • Work under own initiative and have a 'can do' attitude
  • Work to tight deadlines.
  • Use problem solving skills
  • Be flexible / reliable and able to travel between sites.
  • Be confident using IT skills
